Visit Type: Patients have the choice of Phone/Video consultation

Optional: Patients local to our clinic may choose an in person appointment in place of a TeleHealth Appointment when available.

Medical Waiver must be completed prior to purchase, this gives Dr. Jenn a basic understanding of the patients medical history and current concerns.

Appointment Fee is Non-Refundable

Terms of Appointment: MedClub staff will call to schedule a time with each patient after purchase, each patient has the right to reschedule their appointment time once, if they provide 24 hour notice in advance. Appointments may not be cancelled or changed within 24 hours leading up to the selected time. Patients may opt for video conferencing or phone consultation. In person appointments are at the sole discretion of MedClub schedule availability. MedClub reserves the right to terminate any video/voice conference at any time, appointment fee’s are non-refundable.
